Most pathway discussions begin with a diagnosis summary, pathology report, recent imaging reports, treatment history, medication list, and the specific question the patient wants to prepare.
Disease-specific records
Lung cancer may require molecular testing and staging imaging. Gastrointestinal, breast, urologic, head and neck, neurosurgical, lymphoma, and blood cancer cases may need different specialist records.
Translation and file quality
Clear English records are easiest to organize first. If records are in another language, the coordinator may ask which documents need translation before physician or hospital review.
What not to send first
Do not send full imaging files, identity documents, or payment information in the first public form. The team should confirm the secure records step and consent before full records move.
