The first form asks for cancer type, country or region, current treatment status, goal, preferred language, and contact details. It does not ask for full records or imaging files.

How information is used

Information is used to respond to your request, prepare a records checklist, help identify best-fit leading specialist doctors or hospital teams when appropriate, and maintain an internal event history. Clinical, hospital, or partner sharing requires a confirmed pathway and consent.

Sensitive data boundary

Health-derived tags and lead details must stay in the protected backend. They must not be sent to ad pixels, public analytics payloads, URL parameters, or third-party event names.

Withdrawal and deletion requests

Patients or authorized representatives may request contact-stop, consent withdrawal, or deletion review through the official contact channel. The operations team must record the request, verify authority, and apply the retention policy before any further sharing.
