Health data consent
Health data consent allows the team to process the short summary for case coordination. Additional hospital sharing requires a separate consent step.
Consent withdrawal
A patient or authorized representative can ask to withdraw records-sharing consent. After withdrawal is recorded, no doctor, hospital, or partner-pathway sharing should happen unless a new consent is captured.
Representative attestation
Caregivers, family members, or referring physicians must confirm they are authorized to contact the service on the patient's behalf.
